Attention Parents for incoming 7th grade students. Remember your student will need to have proof of additional immunizations for the 2013-2014 7th grade school year. Don’t delay... update your student’s health record as soon as possible, he/she will need to have this turned in prior to the first day of school.
Required NEW immunizations are meningocoocal vaccine, Tdap and 2nd varicella vaccine.
7th Grade
Polio - 4 doses with one after the 4th birthday
DTaP - 3 doses with one after 4th birthday
Tdap/DT - booster within 5 years of the last dose
Measles - 2 doses with first after the 1st birthday
Rubella - 1 dose after the 1st birthday
Mumps - 1 dose after the 1st birthday
Hepatitis B - 3 doses
Varicella - 2 doses
Meningococcal - 1 dose
