Medicaid Notice
Dear Parent/Guardian/Adult Student:
In an effort to provide the best services possible, Channelview ISD has been approved as a provider for School Health and Related Services (SHARS) for participation in the Texas Medicaid program, to assist in recovering funds for services that are already being provided. If you are currently covered under the Medicaid program, Channelview ISD will seek Medicaid reimbursement from the Texas Medicaid Assistance Program (Medicaid) for providing your child with any of the following services as determined by the Admission Review and Dismissal Committee in your child’s individual education plan. Examples of services in the IEP are below:
Personal Care Services Audiology Counseling/ Psychological services Transportation
Medical/School Health Services Occupational therapy Physical therapy Speech therapy

CISD is required to obtain your consent before providing personally identifiable information to Medicaid in order to bill for services the district is providing to your student.
CISD will continue to provide all services outlined in the IEP for your child, at no cost to you, regardless of whether you provide consent for the district to bill Medicaid for services to your student or not.
CISD may not use your child’s benefits under a public benefits or Medicaid program if that use would:
decrease available lifetime coverage or any other insured benefit; or
result in your family paying for services that would otherwise be covered by the public benefits or insurance program and that are required for the child outside of the time that child is in school; or
increase premiums or lead to the discontinuation of benefits or insurance; or
risk loss of eligibility for home and community-based waivers, based on aggregate health-related expenditures.
consent is voluntary and may be revoked at anytime. However. I understand that revocation is not retroactive (i.e. it does not negate an action that has occurred after the consent was given and before the consent was revoked).
