• What to Bring:

  • Proof of Residency – must be valid and enrolling legal guardians name (Light, Trash, Water, Gas or Lease/Deed of Trust)

  • Student SS (is applicable)

  • Student Immunization Record

  • Student BC

  • Enrolling Guardian Valid Government Issued ID

  • Any previous school records

  • Any court orders if applicable

Back-2-School Form (previously known as Annual Update) – CISD students ending the 24-25 school year in CISD

Inscripción de Nuevos Estudiantes - Nuevos Residentes de Channelview

Que Traer:

• Prueba de Residencia - debe ser válida y el nombre del tutor legal de inscripción (Luz, Basura, Agua, Gas o Arrendamiento / Escritura de Fideicomiso)

• SS del Estudiante (es aplicable)

• Registro de Vacunación del Estudiante

• Acta de nacimiento del Estudiante

• Identificacion oficial vigente del tutor legal que incribe al estudiante

• Cualquier registro escolar anterior

• Cualquier orden judicial si es aplicable
